Our story
Founded in Brooklyn. Grown into a city-wide mission.
HealthForce 365 was founded in Brooklyn to serve the communities around us — older adults first, then parents, then children — anyone who needed more than medicine alone could provide.
What began as a service for older adults has grown into a full-population company. Today, we serve pregnant mothers, new parents, children with chronic conditions, adults managing illness, and seniors aging in place — anyone in New York City who needs help connecting health care to the things that make health possible: stable housing, nutritious food, safe transportation, and a caring guide through the system.

Medicaid-Covered Care
HealthForce 365 delivers social care services covered through New York Medicaid — connecting health care to the housing, food, transportation, and support that make health possible.

No Cost to You. Ever.
Services are covered through your New York Medicaid plan. You will never pay out of pocket for our support, and receiving our services does not affect any other benefits you currently receive — SNAP, WIC, Section 8, or public housing.

Bilingual, Local, Compassionate
Our staff speaks the languages of NYC communities and comes from the neighborhoods we serve. We treat every member with dignity — not as a case number. Interpreter services are available at no cost for all other languages.
