What’s included

Nutrition counseling and education
Individual or group sessions with a Registered Dietitian to learn how food supports your health — managing chronic illness, eating well during pregnancy, or building lifelong skills for the whole family.
Medically tailored meals or food prescriptions
Meals designed for your health condition by a Registered Dietitian, or a food prescription — groceries delivered to your home or a voucher for approved stores and farmers’ markets. For members with conditions such as diabetes, heart failure, kidney disease, COPD, hypertension, cancer, asthma, sickle cell, or HIV, and for pregnant or postpartum mothers.
Pantry restocking
Regular deliveries of fresh produce and groceries for pregnant or postpartum mothers and high-risk children under 18. Up to ten family members under a single referral. Available through pregnancy plus 12 months after delivery.
Cooking supplies
If you don’t have what you need to prepare meals at home, we can provide kitchenware, a microwave, or a refrigerator at no cost.

Do you have New York Medicaid?
You must be enrolled in Medicaid Managed Care. If you have Medicaid Fee-for-Service or no insurance, we can still help connect you to community resources.

Do you have unmet social needs?
Housing instability, food insecurity, transportation barriers, utility challenges, employment or education needs, or personal safety concerns.

Not sure if you qualify?
Don't worry — call us or fill out a short form. Our team will walk you through the process in under 30 minutes. No paperwork required to start.

Even if you don't qualify for Enhanced services, We can still connect you to SNAP, WIC, NYCHA, Section 8, food pantries, and other community programs. No one leaves without options.

Your Social Care Navigator creates a personalized plan connecting you to the right services for your situation, health needs, and family.

Receive Services
Medically tailored meals, home safety changes, rides to your social care services, rental assistance — services come to you.

Stay Supported
Your Navigator stays with you, checks in regularly, and adjusts your plan as your needs change. You're never on your own.

Most members can complete a screening within 1–2 business days. Services typically begin within 2–4 weeks of completing your Eligibility Assessment.
